Privacy Notice

Last updated 31 August 2026 · Applies to physical therapy, personal training and app-based coaching provided by Archangel Rehab & Fitness LLC.

This explains what information Archangel holds about you, where it lives, who can see it, and what you can ask us to do with it. It is written to be read rather than to be legally impressive. If anything here is unclear, ask and I will explain it.

Who is responsible for your information

Daniel J. Feliciano, PT, DPT, sole owner and sole treating clinician of Archangel Rehab & Fitness LLC. There is no other clinician. If you want to raise something about your records, you are talking to the person who keeps them.

One thing worth stating plainly: Archangel is a cash-pay practice and does not submit claims to insurers. Nothing about your care is sent to an insurance company by us. If you choose to seek out-of-network reimbursement, you receive a superbill and you submit it yourself — which means you decide what your insurer learns, and when.

What is collected

Where it is kept

Your chart is a document in Archangel's Google Workspace account, in a folder specific to you. Scheduling and payment records are kept in the same account. Access is limited to the practice.

Payments are processed by Stripe. Archangel never sees or stores your full card number. App programming and in-app messages are held by the app provider under their own terms.

Who can see it

Your information is not sold, and it is not used for advertising.

Photographs and video

Photographs or video for progress tracking and movement review stay in your chart and are used for your care. Anything used publicly — marketing, social media, a testimonial — requires separate written permission, is entirely optional, and refusing changes nothing about your care. You can withdraw that permission in writing at any time, though it cannot reach material already published.

Email, text and video visits

Ordinary email and text messages are not encrypted. If you ask us to communicate that way, there is a small risk someone else could read them. Say the word and we will keep clinical detail to phone or in person instead.

Telehealth visits use Google Meet. Sessions are not recorded.

What you can ask for

How long it is kept

Physical therapy records are kept for at least five years after your last visit, as Florida Rule 64B17 requires. Good Faith Estimates are kept for six years, which is what the No Surprises Act requires. Payment records are kept as long as tax rules require.

If something goes wrong

If your information is ever exposed in a way it should not have been, you will be told — what happened, what was involved, and what is being done about it.

Changes to this notice

If this changes, the updated version is posted here with a new date. The version in force is the one on this page.
