What we do

Two core services, one plan.

One of them is healthcare and one of them isn't. I keep that line clear — because it matters to you, and to your insurer.

Restore
Physical Therapy

Licensed clinical care for pain, injury, and movement limitation, led by a Doctor of Physical Therapy.

  • Neck, shoulder, back, hip, knee and ankle pain
  • Orthopedic and sports rehabilitation
  • Post-surgical recovery and return to activity
  • Chronic pain and mobility limitations
  • Balance, gait and movement deficits
  • Objective strength, mobility and range-of-motion testing
  • Home exercise programming and progressions
  • Mobile visits and telehealth support
Optimize
Personal Training

Strength and conditioning for every stage of life. Not healthcare, and I'm explicit about that.

  • In-person and virtual training options
  • Strength development and muscle building
  • Conditioning, mobility and movement quality
  • Sport and lifestyle performance
  • Customized programming and progressions
  • Accountability and habit support
  • Exercise video library access
Objective, not guesswork

Your evaluation uses the Activforce2 digital assessment system to measure joint mobility, muscle strength and movement performance. The plan is built on numbers we can re-test — not on how things looked from across the room. You'll see the same data I do.

Why cash-pay

I don't bill insurance. That's the point.

In an insurance-based clinic, an insurer decides how many visits you get, what counts as medically necessary, and when you're done — often while your therapist is treating two other people in the same hour.

01

The hour is yours

One person, one therapist, sixty minutes. No aides running your exercises while I'm in the next room.

02

You decide when you're done

No visit minimums, no prepaid packages, no contract. Come back when it's useful and stop when it isn't.

03

You'll know the cost first

A written Good Faith Estimate before your first visit, covering the whole expected course of care.

04

Superbills on request

An itemized receipt with the diagnosis and procedure codes your insurer needs, so you can seek out-of-network reimbursement yourself. Superbills apply to qualifying physical therapy only — not training, coaching, nutrition, app access or memberships.

To be straight with you: reimbursement is between you and your insurer, and it's never guaranteed. Call the number on your card and ask what your plan pays for out-of-network outpatient physical therapy before you count on it.

Pricing

What it costs.

Published, because you shouldn't have to book a call to find out whether you can afford it. Three steps: start with a conversation, add ongoing support if you want it, book sessions when you need them.

Step one — Start here

Everyone begins with a conversation. Nobody pays for anything until we both agree it makes sense.

Discovery Call
$0
15 minutes, by phone
We work out whether I'm the right fit before anyone pays anything. If I'm not, I'll tell you who is.
Physical Therapy Evaluation
$175
One-time, in person
Full examination with objective Activforce2 strength and mobility testing, your findings explained plainly, and a written plan.
Personal Training Consultation
$95
One-time
Goals, habits, training history and the kind of support that will actually keep you consistent. Not healthcare.
Step two — The Archangel app (optional)

Everything between your sessions. This is where the plan actually lives — and it is the difference between a program you follow and a program that adapts to you.

Recommended
Archangel App Membership
$149 / month
With the Archangel app
The premier experience, built around you:
  • Your exercise program delivered in full detail — sets, reps, tempo, video demonstration, and written cues for every movement
  • Progress tracked as you go, so you can see what is actually changing
  • Nutrition planning built in — customized calorie and macronutrient targets, tracked in the app or MyFitnessPal, reviewed and adjusted weekly
  • Everyday habit guidance alongside it, so intake is measured against a goal rather than guessed at
  • Direct message me in the app for questions, modifications, or anything you need
  • A reply within 24 hours
Physical therapy is billed separately. The membership covers programming, nutrition and habit support, and app-based access. Clinical physical therapy is not included in it.
Without the app
$0 / month
Sessions only
Still good care. Just less of it between visits:
  • Your home exercise program delivered as a PDF
  • No nutrition planning, and no habit or lifestyle guidance
  • No progress tracking between visits
  • Reach me by business phone, email or text — not immediate, and typically longer than 24 hours

Nutrition and habit guidance is educational. It is not medical nutrition therapy and does not diagnose or treat any medical condition. Anyone whose condition requires medical dietary management is referred to the appropriate licensed provider.

Step three — Sessions as you need them

For established clients. Book a session whenever you want hands-on treatment, coaching, or a change to your plan. No minimum, no package to buy up front.

SessionRate
In-person physical therapy treatment In your home, your gym, or wherever suits you$125
Telehealth physical therapy treatment Same clinician, same hour, over video$95
In-person personal training Not healthcare$95
Virtual personal training Not healthcare$65

Travel within our service area is included. Payment is due at the time of service. Before your first visit you'll receive a written Good Faith Estimate covering your whole expected course of care.

Before you book

A few situations I can't help with.

I'd rather tell you now than after you've booked.

If you have Medicare

Archangel is not a Medicare provider, and federal rules don't allow me to see Medicare beneficiaries for physical therapy on a cash-pay basis — even if you'd rather pay privately.

If you need physical therapy, look for a Medicare-enrolled provider. I'm glad to point you somewhere useful.

If your injury happened at work

Work-related injuries go through your employer's workers' compensation process. That system exists to make sure your care is covered and your rights are protected, and a cash-pay superbill is not a substitute for it. Start with your employer.

Auto accident or liability claim

Care connected to a motor vehicle accident or another liability claim usually needs to be coordinated with that claim. Let's talk before you book, so nothing here complicates your case.

About

"People deserve honest guidance, clear information about their bodies, and the freedom to make confident decisions about their health."

Archangel was built on that belief. Every relationship starts with understanding where you are and where you want to go — then clear, evidence-based recommendations and a path that fits your life.

My role is to guide, educate and support. Without pressure, unnecessary visits, long-term commitments, or preset limits on your progress.

The name reflects the mission. Throughout Scripture, angels appear as messengers, protectors, and servants of God's purpose. In that spirit, this practice exists to provide guidance, support and accountability so people can move forward with clarity, confidence and strength.

Health is a form of stewardship. Through intentional movement, strength, discipline and lifestyle, lasting change is possible — not just improvement, but transformation that strengthens individuals, families and the community.

Your rights as a self-pay patient

You have the right to know what your care will cost.

Under the No Surprises Act, providers must give patients who don't have insurance — or who aren't using it — a written estimate before care begins.

  • You have the right to a Good Faith Estimate for the total expected cost of any non-emergency items or services.
  • You can ask for one before scheduling anything. Simply asking what something will cost counts as a request.
  • If you schedule care, you'll receive a written estimate at least one business day before your appointment.
  • If you are billed at least $400 more than your estimate, you can dispute the bill.
  • Keep a copy or a photograph of your estimate.

For more about this right, visit cms.gov/nosurprises or call 1-800-985-3059.

To request one from Archangel: email daniel.feliciano@archangelrehabandfitness.com or call (850) 312-1284. You'll have it in writing within three business days, usually the same day.
